WP 6.2+, PHP 7.4+

Forge Forms is WebSite Relief’s self-hosted WordPress form builder. It has no licenses and no paid add-ons. You build forms with drag and drop, and every submission is saved as an entry you can search and export.

Install

Upload forge-forms.zip in Plugins → Add New → Upload Plugin, then activate it. Requires WordPress 6.2+ and PHP 7.4+.

Use

  1. Go to Forge Forms → Add New Form. Name the form and drag fields in. Click a field to edit it.
  2. Use the Settings and Notifications tabs, then click Publish.
  3. Put [forge_form id="123"] on any page. This works in Divi, Elementor and the classic editor. You can also use the Forge Form block.
  4. Submissions appear under Forge Forms → Entries, where you can view, search, delete or export them to CSV.

Features

  • 15 field types: text, paragraph, email, phone, website, number, date, dropdown, radio, checkboxes, consent, file upload, hidden, HTML block and page break.
  • Conditional logic: show or hide a field when all or any rules match. The server enforces it too, so hidden fields are never saved.
  • Multi-step forms with a progress bar.
  • Half-width fields for side-by-side layouts.
  • File uploads with type and size limits. Files are stored in random folders and downloaded only through a permission check.
  • Unlimited email notifications with merge tags ({field_key}, {all_fields}, {form_title}, {entry_url}, …).
  • Webhooks: each submission is POSTed as JSON to Zapier, Make, n8n, a CRM, and so on.
  • Spam protection: a honeypot, a signed minimum-time token, and optional Cloudflare Turnstile (free).
  • AJAX submit with inline errors, and a fallback that still works without JavaScript.
  • Message or redirect confirmations.
  • An optional uninstall cleanup that removes all forms, entries and files.

Styling

Forms inherit the theme’s fonts. To brand a form, override the CSS variables:

.ff-wrap { --ff-accent: #c2410c; --ff-radius: 0; }

Developer hooks

Action: forgeforms_submission

Filters: forgeforms_validation_errors, forgeforms_entry_data, forgeforms_webhook_payload, forgeforms_is_spam, forgeforms_form_html, forgeforms_capability, forgeforms_client_ip

Notes

  • For reliable email delivery, use an SMTP plugin or service (Postmark, SES, Mailgun) with a From address on your own domain.
  • On nginx hosts, .htaccess is ignored. Uploaded files are still protected by their unguessable folder names. For stricter protection, add a deny all rule for /wp-content/uploads/forgeforms/.
